Chronic cough, defined as a cough lasting more than eight weeks, is a common and often challenging condition to manage in clinical practice. It significantly affects patients' quality of life and may indicate underlying serious diseases.
A thorough history and physical examination are the first steps in diagnosing chronic cough. Laboratory tests, imaging, and pulmonary function tests may also be necessary. The most common causes are upper airway cough syndrome (UACS), gastroesophageal reflux disease (GERD), and asthma. However, less common causes such as chronic obstructive pulmonary disease (COPD), lung cancer, and interstitial lung diseases should not be overlooked.
Treatment should be directed at the underlying cause. Empirical treatment for UACS, GERD, and asthma can be considered in the absence of alarming features. Refractory cases may require further investigations such as bronchoscopy or esophageal pH monitoring. Non-pharmacological interventions, including smoking cessation and avoidance of cough triggers, are crucial components of management.
Chronic cough is a complex condition requiring a comprehensive and systematic approach for diagnosis and management. Understanding the common and less common causes can guide clinicians in their investigations and treatment strategies. This approach ensures optimal patient care and reduces unnecessary investigations and treatments.
