Primary constipation can be further divided into normal transit, slow transit, and defecatory disorders, each with distinct pathophysiological mechanisms.
Diagnosis is primarily based on clinical criteria, such as the Rome IV criteria. However, healthcare professionals should also consider the patient's medical history, physical examination, and, if necessary, additional tests like colonoscopy or anorectal manometry, especially in patients with alarm symptoms or refractory constipation.
Management should be individualized, considering the patient's symptoms, preferences, and the underlying pathophysiology. Initial management usually involves lifestyle modifications, dietary changes, and over-the-counter laxatives. For patients with refractory constipation, newer pharmacological agents like prosecretory agents or biofeedback therapy for defecatory disorders can be considered.

They should provide patient education, reassurance, and counseling about the chronic nature of constipation and the importance of adherence to treatment. Regular follow-up is essential to assess treatment response and adjust management as needed.
