Thyroid nodules, prevalent in up to 65% of the population, pose a significant diagnostic challenge for healthcare professionals. Their management requires a nuanced understanding of their pathophysiology, diagnostic modalities, and treatment options. This guide aims to provide a comprehensive overview of thyroid nodule evaluation.
Thyroid nodules are more common in women, the elderly, and individuals with a history of radiation exposure. While most are asymptomatic, some may present with compressive symptoms or thyroid dysfunction.
Initial evaluation typically involves thyroid ultrasound, which provides valuable information about nodule size, echogenicity, and vascularity. Fine-needle aspiration (FNA) biopsy, guided by ultrasound, can further classify nodules as benign, malignant, or indeterminate.
The Bethesda System for Reporting Thyroid Cytopathology is widely used to interpret FNA results. It categorizes nodules into six classes, each with an associated malignancy risk and recommended management strategy.
Management options range from surveillance to surgery, depending on the nodule's Bethesda classification, size, and patient's clinical context. For example, small benign nodules may be monitored, while larger or malignant ones typically necessitate surgery.
For indeterminate nodules, molecular testing can provide additional information to guide management. Tests such as Afirma, ThyroSeq, and RosettaGX reveal can help predict malignancy and inform the decision between surveillance and surgery.
Thyroid nodule evaluation is a complex process that requires a detailed understanding of various diagnostic and management strategies.
