Fever, an elevation in body temperature, is a common symptom in clinical practice. It is a complex physiological response to disease, often indicating an underlying infection. Understanding the pathophysiology, differential diagnosis, and management of fever is crucial for healthcare professionals.
Fever is the result of an upward shift in the hypothalamic set point, instigated by pyrogens. These pyrogens stimulate the production of prostaglandin E2 (PGE2), which acts on the hypothalamus, leading to an increase in body temperature.
Identifying the cause of fever requires a comprehensive approach. It could be due to infectious diseases, non-infectious inflammatory conditions, malignancies, or even drug reactions. A detailed history, physical examination, and appropriate investigations are essential for accurate diagnosis.
Treatment of fever primarily involves managing the underlying cause. Antipyretics can be used to provide symptomatic relief. However, the use of antipyretics should be judicious, considering their potential side effects.
Decoding fever requires a deep understanding of its pathophysiology, causes, and management strategies. As healthcare professionals, our role extends beyond treating the symptom. It involves identifying the underlying cause and managing it effectively to ensure optimal patient outcomes.
