Unexplained weight loss (UWL), defined as a loss of more than 5% of body weight over a period of 6 to 12 months without a known cause, is a common yet perplexing clinical scenario. It is often a harbinger of serious underlying pathology and requires thorough evaluation. This article aims to provide a comprehensive approach to the evaluation of UWL in clinical practice.
UWL can be attributed to a multitude of causes, broadly classified into three categories: malignancies, non-malignant organic diseases, and psychiatric disorders. Malignancies, particularly gastrointestinal and lung cancers, are the most common cause. Non-malignant organic diseases such as gastrointestinal disorders, endocrine diseases, and chronic infections also contribute significantly. Psychiatric disorders like depression and eating disorders are often overlooked but are vital considerations.
A systematic approach is essential in evaluating UWL. A detailed history and physical examination often provide valuable clues. The history should focus on dietary habits, physical activity, associated symptoms, and psychosocial factors. The physical examination should be comprehensive, with an emphasis on signs of malignancy, malabsorption, and endocrine disorders. Basic investigations include complete blood count, metabolic panel, thyroid function tests, and stool examination.
Imaging plays a pivotal role in the diagnostic workup of UWL. The choice of imaging modality depends on the clinical context, but generally, computed tomography of the abdomen and chest is the first-line investigation. Special investigations such as endoscopy, colonoscopy, and biopsy may be warranted based on the initial findings. In cases where the cause remains elusive, a watchful waiting approach can be adopted, with periodic reassessment and further investigations as indicated.
Management is directed towards the underlying cause. In cases where no cause is identified, symptomatic treatment and nutritional support should be provided. Regular follow-up is crucial to monitor the patient's weight, assess response to treatment, and detect any new symptoms or signs that may point to a specific diagnosis.
UWL is a challenging clinical problem that requires a systematic and comprehensive approach. A thorough history, physical examination, and appropriate investigations are key to identifying the underlying cause. Regular follow-up is essential, and a multidisciplinary approach involving dietitians, psychologists, and other specialists may often be required to manage these patients effectively.
