Enteric fever, predominantly caused by Salmonella Typhi and Paratyphi, remains a significant health concern worldwide. This article provides an overview of the effective strategies for the diagnosis and management of this condition.
Early diagnosis is crucial in enteric fever management. Blood cultures remain the gold standard, with sensitivity ranging from 40-80% in the first week of illness. However, the sensitivity decreases as the disease progresses. Bone marrow cultures have higher sensitivity but are invasive. Serological tests like the Widal test are widely used but have variable sensitivity and specificity.
Antimicrobial therapy is the cornerstone of enteric fever treatment. Choice of antibiotics should be guided by local susceptibility patterns. Fluoroquinolones were once the first-line treatment, but increasing resistance has shifted preference to third-generation cephalosporins and azithromycin. Patients with severe disease may require ceftriaxone or cefotaxime. In cases of multidrug-resistant strains, newer agents like typhoid conjugate vaccine (TCV) have shown promising results.
Besides antimicrobial therapy, supportive care plays a significant role in managing enteric fever. Hydration, correction of electrolyte imbalances, and nutritional support are essential. In severe cases, intensive care may be required.
Vaccination is an effective preventive measure. Two vaccines are currently available: the oral live-attenuated Ty21a vaccine and the injectable Vi polysaccharide vaccine. The recently introduced TCV has shown high efficacy and longer duration of protection.
In conclusion, effective management of enteric fever requires early diagnosis, appropriate antimicrobial therapy, supportive care, and vaccination. Continued research and development of newer diagnostic methods and treatments are essential to combat this global health issue.
