Initial investigations should include complete blood count, blood culture, and specific serological tests based on clinical suspicion. Where indicated, skin biopsy, lumbar puncture, or imaging studies may be required to reach a definitive diagnosis.
The differential diagnoses of fever with rash are vast. Infectious causes include viral exanthems, bacterial infections, and vector-borne diseases. Autoimmune conditions, drug reactions, and malignancies should also be considered. The temporal relationship between fever and rash onset can provide valuable diagnostic clues. For instance, a rash following fever suggests infectious mononucleosis or Kawasaki disease, while a rash preceding fever may indicate drug reactions or autoimmune diseases.
Management is directed by the underlying cause. For infectious diseases, appropriate antimicrobial therapy is required. In contrast, autoimmune conditions may necessitate immunosuppressive therapy, and drug reactions require discontinuation of the offending agent. Supportive care, including antipyretics and hydration, is essential in all cases.

By considering a broad range of differential diagnoses and understanding key clinical patterns, clinicians can effectively navigate this diagnostic challenge and provide appropriate patient care.
