Acute chest pain (ACP) is one of the most common symptoms encountered in emergency departments. It poses a significant diagnostic challenge due to its diverse etiology, ranging from benign conditions to life-threatening emergencies such as acute coronary syndrome (ACS).
Initial evaluation should include a thorough history and physical examination to identify high-risk features suggestive of ACS. The use of risk stratification tools, such as the HEART score, can aid in decision-making. Electrocardiogram (ECG) and cardiac biomarkers (troponin) are essential in the initial assessment.
Further diagnostic testing depends on the initial risk assessment. Low-risk patients may undergo stress testing or advanced imaging. High-risk patients should be considered for immediate invasive strategies. Coronary computed tomography angiography (CCTA) has emerged as a valuable tool in the evaluation of intermediate-risk patients.
Management strategies are guided by the underlying cause. ACS requires urgent revascularization, either by percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG). Non-cardiac causes of ACP should be managed accordingly, with appropriate referral to specialists when necessary.
Disposition decisions should be individualized, based on the patient's risk profile, diagnostic evaluation, and response to initial management. High-risk patients warrant hospital admission, while selected low-risk patients may be safely discharged with close outpatient follow-up.
ACP represents a complex clinical scenario requiring a systematic approach to ensure appropriate diagnosis and management. Through a combination of clinical assessment, risk stratification, diagnostic testing, and tailored management strategies, healthcare professionals can effectively navigate the challenges posed by this common, yet potentially serious, clinical presentation.
