Anemia, a global health concern, is a condition marked by a decrease in the total amount of red blood cells or hemoglobin in the blood. With its multifaceted etiology, diagnosis can be complex, demanding a comprehensive understanding of its diverse causes and manifestations.
The initial evaluation of anemia involves complete blood count (CBC) and reticulocyte count. The CBC provides valuable information about hemoglobin levels, red cell indices, and platelet count. The reticulocyte count helps determine whether the bone marrow response to anemia is adequate or not.
Anemia can be classified based on the mean cell volume (MCV) into microcytic, normocytic, and macrocytic anemias. This classification aids in narrowing down the differential diagnosis and guiding further investigations.
After the initial evaluation and classification, further investigations may be needed based on the suspected cause. These may include iron studies, vitamin B12 and folate levels, hemoglobin electrophoresis, bone marrow examination, or other specific tests.
Special considerations should be given to certain populations like the elderly, pregnant women, and individuals with comorbidities. These groups may require additional tests and a more nuanced approach to diagnosis and management.
Unraveling the complexity of anemia requires a systematic approach, starting from a thorough history and physical examination, followed by laboratory investigations. A keen understanding of the pathophysiology, combined with a methodical approach, can lead to accurate diagnosis and effective treatment, ultimately improving patient outcomes.
