Acute chest pain poses a diagnostic challenge to clinicians due to its diverse etiology. It is crucial to promptly identify life-threatening conditions such as acute coronary syndrome (ACS) and aortic dissection, while also considering other non-cardiac causes.
The initial assessment of a patient presenting with acute chest pain should involve a thorough history, physical examination, electrocardiogram (ECG), and cardiac biomarkers. The HEART score is a useful tool for risk stratification, incorporating History, ECG, Age, Risk factors, and Troponin level.
Beyond the initial evaluation, further diagnostic testing may be required based on the patient's risk stratification. These can include stress testing, coronary computed tomography angiography (CCTA), or invasive coronary angiography. The choice of test should be guided by the patient's pre-test probability of ACS and their ability to exercise.
Management should be tailored to the underlying cause. ACS requires urgent revascularization, either through percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG). Non-cardiac causes may require a multidisciplinary approach involving specialists from gastroenterology, pulmonology, or musculoskeletal medicine.
Acute chest pain is a complex clinical scenario requiring a comprehensive approach. Clinicians should use a systematic process of initial evaluation, risk stratification, diagnostic testing, and tailored management. By doing so, they can ensure the timely identification and treatment of life-threatening conditions while also addressing other potential causes of chest pain.
