Initial evaluation should include a detailed history, focusing on risk factors such as radiation exposure, family history of thyroid cancer, and symptoms suggestive of malignancy like hoarseness, dysphagia, and neck pain. Physical examination should assess nodule size, consistency, and the presence of lymphadenopathy.
High-resolution ultrasound is the imaging modality of choice for evaluating thyroid nodules. It provides information on nodule size, echogenicity, margins, presence of calcifications, and vascularity, all of which can help predict malignancy.
Fine-needle aspiration (FNA) biopsy is the most reliable method for determining the nature of a thyroid nodule. The Bethesda System for Reporting Thyroid Cytopathology is widely used to classify FNA results, guiding further management.
For nodules with indeterminate cytology, molecular testing can be useful. Tests, such as the Afirma Gene Expression Classifier and Thyroseq, can help to identify nodules that are likely benign, reducing unnecessary surgeries.
Management options range from observation for benign nodules to surgery for malignant or symptomatic nodules. Radioactive iodine or ethanol ablation may be options for patients who are not surgical candidates.
In summary, the comprehensive evaluation of thyroid nodules involves a multi-step process that includes history, physical examination, radiological assessment, cytological evaluation, and possibly genetic testing. A systematic approach can help to identify those nodules that require intervention, while avoiding unnecessary procedures in patients with benign disease.
