Initial evaluation of thyroid nodules involves a thorough history and physical examination, focusing on risk factors for malignancy. High-resolution ultrasound (US) is the primary imaging modality for nodules, providing information on size, composition, echogenicity, and vascularity. Fine-needle aspiration (FNA) biopsy is the gold standard for cytologic evaluation, with molecular testing aiding in indeterminate cases.
Risk stratification is crucial in determining the management of thyroid nodules. The American Thyroid Association (ATA) has developed a risk stratification system based on US features. It classifies nodules into five categories (benign, very low suspicion, low suspicion, intermediate suspicion, and high suspicion), guiding the decision for FNA biopsy.
Management of thyroid nodules depends on the risk stratification and patient's individual characteristics. Observation is appropriate for benign nodules, while surgery is indicated for malignant or suspicious nodules. For indeterminate nodules, repeat FNA, molecular testing, or lobectomy may be considered. Radioactive iodine and thyroid hormone suppression therapy are other options for specific cases.
Follow-up is essential to monitor the nodule for changes in size or appearance, or the development of new symptoms. US is the preferred follow-up modality, with the frequency determined by the initial risk category. Any significant change warrants repeat FNA or consideration for surgery.
