The management of peptic ulcer disease (PUD) has undergone significant changes over the past few decades, thanks to advancements in diagnostic techniques, pharmacological therapies and a better understanding of its pathophysiology. This article provides a comprehensive review of these advancements for healthcare professionals.
Advancements in endoscopic technology have revolutionized the diagnosis of PUD. High-definition endoscopy, narrow-band imaging, and chromoendoscopy have improved the visualization of the mucosal surface, aiding in the detection and characterization of ulcers. Moreover, rapid urease tests and urea breath tests have enhanced the detection of Helicobacter pylori, the leading cause of PUD.
Proton pump inhibitors (PPIs) remain the cornerstone of PUD treatment. However, recent advancements have seen the emergence of vonoprazan, a potassium-competitive acid blocker, which exhibits superior acid-suppressing effects compared to traditional PPIs. Additionally, the advent of quadruple therapy, including bismuth, has shown increased eradication rates of H. pylori, thereby reducing ulcer recurrence.
A better understanding of the pathophysiology of PUD has also contributed to improved management. The discovery of the role of H. pylori and the influence of non-steroidal anti-inflammatory drugs (NSAIDs) on gastric mucosal integrity have led to targeted treatments. Furthermore, research into the genetic predisposition to PUD is paving the way for personalized medicine approaches.
In conclusion, the management of PUD has significantly improved due to advancements in diagnostic techniques, pharmacological therapies, and an enhanced understanding of its pathophysiology. Continued research and innovation in these areas promise to further refine PUD management, leading to better patient outcomes.
