Its etiology is multifactorial, and its identification and management can often be challenging. This article aims to provide a comprehensive guide to recognize the red flags and establish a diagnostic approach to anemia.
Early detection of anemia relies on the identification of 'red flags' - symptoms that may indicate a serious underlying condition. These include fatigue, pallor, dyspnea, and tachycardia. More concerning signs such as chest pain, syncope, and signs of heart failure should prompt immediate further investigation. It's crucial to remember that anemia might be a manifestation of a systemic disease, thus a thorough clinical history and physical examination are paramount.
The first step in the diagnostic approach involves laboratory testing, primarily complete blood count (CBC) and reticulocyte count. The CBC provides valuable information about the red blood cell indices which can classify anemia as microcytic, normocytic, or macrocytic, guiding further investigations. The reticulocyte count helps in distinguishing between underproduction and increased destruction or loss of red blood cells.
Further diagnostic tests should be guided by the clinical context and initial laboratory findings. Iron studies, vitamin B12, and folate levels can help identify nutritional deficiencies. Hemolysis can be assessed by lactate dehydrogenase, haptoglobin, and direct Coombs test. In cases of suspected bone marrow failure or infiltration, bone marrow biopsy may be warranted.

Recognizing the red flags and adopting a methodical, comprehensive diagnostic approach is key to identifying the underlying cause and initiating appropriate management.
