Interstitial lung diseases (ILDs) are a heterogeneous group of disorders characterized by inflammation and fibrosis of the pulmonary interstitium. This article provides a comprehensive overview of these complex diseases.
ILDs can be idiopathic or associated with systemic diseases, environmental exposures, or medications. Idiopathic pulmonary fibrosis (IPF) is the most common ILD and is characterized by progressive fibrosis without a known cause. Other ILDs include sarcoidosis, hypersensitivity pneumonitis, and connective tissue disease-associated ILDs.
Patients typically present with progressive dyspnea and nonproductive cough. Physical examination may reveal crackles and clubbing. High-resolution computed tomography (HRCT) is crucial for diagnosis. Pulmonary function tests often show a restrictive pattern with reduced diffusion capacity.
Management strategies depend on the underlying cause. For IPF, antifibrotic agents such as pirfenidone and nintedanib can slow disease progression. In cases of ILD associated with connective tissue diseases, immunosuppressive therapy is often beneficial. Lung transplantation may be considered in advanced cases.
The prognosis varies widely, depending on the specific type of ILD. IPF typically has a poor prognosis with a median survival of 2-3 years from diagnosis. On the other hand, ILDs associated with connective tissue diseases often have a more favorable prognosis.
In conclusion, ILDs are a diverse group of diseases with varying etiologies, clinical presentations, and prognoses. A thorough understanding of these diseases is essential for their effective management.
