Initial assessment should focus on obtaining a detailed patient history. Important factors include the onset, location, duration, character, and radiation of pain, as well as any associated symptoms. This information aids in distinguishing between visceral, parietal, and referred pain, thus narrowing down potential diagnoses.
A thorough physical examination should follow the patient's history. This involves inspection, auscultation, palpation, and percussion of the abdomen. Findings such as distension, tenderness, or guarding can suggest specific diagnoses such as appendicitis or cholecystitis.
Based on the history and physical examination, appropriate laboratory and imaging studies should be ordered. Complete blood counts, liver function tests, and abdominal ultrasound or CT scans can provide valuable information. For instance, elevated white blood cells may suggest infection, while abnormal liver enzymes could indicate hepatobiliary disease.
In certain populations like the elderly, children, and pregnant women, abdominal pain may present atypically or be masked by other conditions. Extra care should be taken when evaluating these patients, and differential diagnoses should be adjusted accordingly.
Abdominal pain, while common, requires a comprehensive and systematic approach for effective evaluation. By meticulously gathering patient history, conducting a thorough physical examination, and utilizing appropriate diagnostic tools, healthcare professionals can accurately diagnose and manage this complex symptom.
