Peptic ulcer disease (PUD) is a prevalent digestive disorder that requires a well-structured approach for optimal patient outcomes. This article provides an overview of PUD management strategies, emphasizing diagnosis, treatment, and patient education.
Early and accurate diagnosis of PUD is crucial. Physicians should have a high index of suspicion in patients presenting with dyspepsia, abdominal pain, or gastrointestinal bleeding. Diagnostic tools include upper gastrointestinal endoscopy, which allows direct visualization of the ulcer and biopsy for Helicobacter pylori detection, and non-invasive tests such as urea breath test, fecal antigen test, or serology.
Treatment of PUD involves eradicating H. pylori infection, if present, and inhibiting gastric acid secretion. Triple or quadruple therapy is recommended for H. pylori eradication, while proton pump inhibitors are the mainstay for acid suppression. In cases of non-responders, a review of patient compliance, reinfection, or resistance should be undertaken.
Patient education is a cornerstone of PUD management. Healthcare professionals should counsel patients about the importance of medication adherence, the potential side effects of treatment, and the need for follow-up. Lifestyle modifications, including smoking cessation, limiting alcohol and NSAID use, and stress management, can also aid in ulcer healing and prevention.
Regular follow-up is essential to assess treatment response, detect complications, and ensure H. pylori eradication. For patients with uncomplicated PUD, symptom resolution may be an adequate marker of healing. However, endoscopic confirmation should be considered in high-risk patients or those with persistent symptoms.
Optimizing PUD management requires a comprehensive approach encompassing accurate diagnosis, effective treatment, patient education, and regular follow-up. By adhering to these principles, healthcare professionals can significantly improve patient outcomes in PUD.
