Chronic cough, defined as a persistent cough lasting more than eight weeks, is a common condition often presenting diagnostic and management challenges in clinical practice. Despite its high prevalence, the underlying etiology often remains elusive, necessitating a comprehensive, systematic approach for effective diagnosis and management.
The majority of chronic cough cases are attributed to asthma, gastroesophageal reflux disease (GERD), and upper airway cough syndrome (UACS). However, non-acid reflux, non-asthmatic eosinophilic bronchitis, and cough hypersensitivity syndrome are increasingly recognized as significant contributors. Diagnostic evaluation should begin with a detailed history and physical examination, followed by targeted investigations based on clinical suspicion. In cases where common causes are ruled out, more specialized testing may be required.
Treatment should be etiology-specific and tailored to the individual patient. Empirical therapy for the most common causes can be initiated while awaiting diagnostic results. For refractory cases, a multidisciplinary approach involving pulmonologists, gastroenterologists, and otolaryngologists may be beneficial. Non-pharmacological interventions, including behavioral therapy and speech pathology consultation, can also play a crucial role in managing chronic cough.
Recent advancements in the understanding of chronic cough pathophysiology have led to the development of novel therapeutic agents. These include P2X3 receptor antagonists, which have shown promise in clinical trials for refractory chronic cough. Furthermore, the concept of cough hypersensitivity syndrome provides a unified framework for understanding the multifactorial nature of chronic cough and may inform future diagnostic and therapeutic strategies.
Chronic cough presents a significant clinical challenge due to its multifactorial etiology and the potential for significant impact on patient quality of life. A comprehensive, systematic approach to diagnosis and management can facilitate effective treatment and improve patient outcomes. Emerging concepts and therapies hold promise for further enhancing our ability to manage this common and often debilitating condition.
