The initial assessment is critical in the evaluation of abdominal pain. A thorough history taking, including the onset, location, duration, character, and associated symptoms of the pain, can help narrow down the differential diagnoses. The physical examination should focus on abdominal tenderness, distension, guarding or rigidity, and bowel sounds.
Abdominal pain can be categorized as visceral, parietal, or referred. Visceral pain arises from organ distension or inflammation, parietal pain from direct irritation of the parietal peritoneum, and referred pain from distant sites. Differential diagnoses can be vast, ranging from gastrointestinal conditions like appendicitis or peptic ulcer disease to gynecological conditions like ectopic pregnancy or ovarian torsion.
The choice of diagnostic tests depends on the suspected underlying cause. Basic blood tests, including complete blood count, liver function tests, and lipase, can be helpful. Imaging studies such as ultrasound, computed tomography, or magnetic resonance imaging may also be necessary. In some cases, endoscopic procedures can be employed for a more direct evaluation.
The management of abdominal pain is tailored to the underlying cause. This may involve medications for pain relief, antibiotics for infections, or surgical intervention for conditions like appendicitis or cholecystitis. In some cases, referral to a specialist may be necessary.
Evaluating abdominal pain can be complex due to the broad differential diagnosis. A systematic approach, involving a thorough history, physical examination, appropriate diagnostic testing, and tailored management, is essential for optimal patient care.
