Gestational diabetes mellitus (GDM) poses significant risks to both mother and child, necessitating a comprehensive management approach. This article aims to elucidate such an approach to optimize patient outcomes.
Early detection and diagnosis are paramount in managing GDM. Universal screening between 24-28 weeks of gestation using an oral glucose tolerance test is recommended. Additionally, high-risk individuals should be screened at the first prenatal visit.
Medical nutrition therapy (MNT) is the cornerstone of GDM management. It involves individualized meal planning to achieve euglycemia, adequate nutrition, and appropriate weight gain. Regular dietetic consultations are essential to monitor progress and adjust the plan as necessary.
Physical activity complements MNT in achieving glycemic control. Unless contraindicated, pregnant women should aim for at least 150 minutes of moderate-intensity aerobic activity weekly, distributed over at least three days.
If MNT and physical activity fail to achieve glycemic targets within one to two weeks, pharmacological therapy should be initiated. Insulin is the first-line treatment, but oral hypoglycemic agents like metformin and glyburide are increasingly used.
Postpartum follow-up is crucial as GDM increases the risk of type 2 diabetes. A 75g oral glucose tolerance test should be performed at 6-12 weeks postpartum, followed by lifelong screening for diabetes at least every three years.
GDM management requires a comprehensive approach encompassing early detection, MNT, physical activity, pharmacological therapy, and postpartum follow-up. By adopting such a strategy, healthcare professionals can optimize outcomes for both mother and child, mitigating the risks associated with GDM.
