Enteric fever, commonly known as typhoid fever, is a systemic infection primarily caused by Salmonella Typhi and Salmonella Paratyphi A, B, and C. The disease presents a significant health challenge, particularly in areas with inadequate sanitation and poor access to clean drinking water. Salmonella Typhi and Paratyphi are the causative organisms of enteric fever. These bacteria are transmitted via the fecal-oral route, often through contaminated food or water. High-risk areas include South Asia, Africa, and parts of South America. Travelers to these regions are particularly at risk.
Patients typically present with high-grade fever, malaise, abdominal discomfort, and constipation or diarrhea. In severe cases, complications such as intestinal perforation, encephalopathy, or myocarditis may occur. Early diagnosis is critical to preventing these complications.
Diagnosis of enteric fever primarily involves blood culture, bone marrow culture, or stool culture. Serological tests like Widal test are also used, but they have lower sensitivity and specificity. Molecular methods, such as PCR, are increasingly being used for rapid and accurate diagnosis.
Antibiotic therapy is the cornerstone of treatment. Choice of antibiotic is guided by local susceptibility patterns. Fluoroquinolones, third-generation cephalosporins, and azithromycin are commonly used. In resistant cases, carbapenems may be considered. Supportive care includes hydration and antipyretics.
Preventive measures include safe food and water practices, improved sanitation, and vaccination. Two vaccines are currently available: the oral live-attenuated vaccine (Ty21a) and the injectable polysaccharide vaccine (ViCPS).
In conclusion, enteric fever remains a significant global health challenge. Effective management involves early diagnosis, appropriate antibiotic therapy, and preventive measures. Ongoing research into better diagnostic tools and more effective vaccines is crucial for controlling this disease.
