Dyslipidemia can be primary, resulting from genetic factors, or secondary, due to lifestyle or other medical conditions. It usually involves elevated low-density lipoprotein cholesterol (LDL-C), triglycerides (TGs), or decreased high-density lipoprotein cholesterol (HDL-C). These disruptions in lipid metabolism contribute to atherosclerosis and subsequently, cardiovascular diseases.
Assessment involves a thorough medical history, physical examination, and laboratory tests. Lipid profile, including total cholesterol, LDL-C, HDL-C, and TGs, is fundamental. Additional tests like apolipoprotein B and lipoprotein(a) may provide further insights.
Treatment should be individualized, considering the patient's overall cardiovascular risk. Lifestyle modifications form the cornerstone of management, encompassing dietary changes, regular physical activity, and smoking cessation. Pharmacotherapy is indicated for patients at high risk or with severe dyslipidemia. Statins are the first-line drug therapy, while other agents like ezetimibe, fibrates, and PCSK9 inhibitors may be added as needed.
Regular monitoring of lipid levels and patient adherence is essential to assess treatment efficacy and adjust management strategies. Treatment goals are primarily based on LDL-C levels, but other lipid parameters and patient-specific factors also play a role.
Understanding and managing dyslipidemia is a complex but vital aspect of preventing cardiovascular diseases. A comprehensive, patient-centered approach encompassing assessment, treatment, and regular monitoring can optimize outcomes and reduce cardiovascular risk.
