Acute chest pain (ACP) is a prevalent symptom in clinical practice, often indicative of potentially life-threatening conditions such as acute coronary syndrome (ACS) or pulmonary embolism. However, it may also stem from benign causes, necessitating a careful and systematic diagnostic approach.
ACP evaluation commences with a thorough history and physical examination. The aim is to identify high-risk features suggestive of ACS, including pain characteristics, associated symptoms, risk factors, and prior cardiac history. Concurrently, vital signs and physical findings can hint at alternative diagnoses, like aortic dissection or pericarditis.
Electrocardiogram (ECG) is the first-line diagnostic test, performed promptly to detect ST-segment elevation myocardial infarction (STEMI). Non-diagnostic ECGs do not exclude ACS and warrant further testing. Cardiac biomarkers, primarily high-sensitivity troponins, are invaluable in ACS diagnosis, with serial measurements recommended in the first 3 hours from symptom onset.
Chest X-ray, while typically normal in ACS, can reveal alternative diagnoses like pneumonia or pneumothorax. Computed tomography (CT) angiography is the imaging modality of choice for suspected pulmonary embolism or aortic dissection. Coronary CT angiography can also be used in low-risk patients to rule out coronary artery disease.
Risk stratification tools, such as the HEART score, guide further management. Low-risk patients might undergo stress testing or advanced imaging, while high-risk patients require hospitalization and invasive strategies.
ACP presents a diagnostic challenge, necessitating a systematic approach to distinguish serious from benign causes. A comprehensive history and physical examination, combined with judicious use of diagnostic tests and risk stratification tools, are pivotal in guiding management and improving patient outcomes. Continued research and technological advancements promise to refine this process further, enhancing our ability to accurately diagnose and treat this common clinical presentation.
