Chronic cough, defined as a cough lasting more than eight weeks, can be a challenging condition to manage in clinical practice. It can significantly impact a patient's quality of life and often requires a comprehensive, multidisciplinary approach to diagnosis and treatment.
Identifying the underlying cause of chronic cough is a crucial first step. Common causes include gastroesophageal reflux disease (GERD), asthma, and upper airway cough syndrome (UACS) secondary to rhinosinus conditions. Less common etiologies include chronic bronchitis, bronchiectasis, and lung cancer. A detailed history, physical examination, and targeted diagnostic tests are essential to pinpoint the cause.
Treatment should be tailored to the identified cause. For GERD-induced cough, proton pump inhibitors and lifestyle modifications are the mainstay of therapy. Inhaled corticosteroids are typically used for asthma-related cough while UACS may require antihistamines, decongestants, or nasal corticosteroids. In cases where a specific cause cannot be identified, empirical treatment or referral to a specialist may be necessary.
Given the complexities associated with chronic cough, a multidisciplinary approach involving pulmonologists, gastroenterologists, otolaryngologists, and even speech therapists can be beneficial. Speech therapy, in particular, has been shown to reduce cough severity and improve quality of life in patients with chronic cough.
Managing chronic cough requires a comprehensive, etiology-based approach. An accurate diagnosis followed by targeted treatment is key. In challenging cases, a multidisciplinary team can provide additional expertise and support. As healthcare professionals, we must strive to alleviate this burdensome symptom and improve our patients' quality of life.
