Introduction

Enteric fever, comprising typhoid and paratyphoid fever, remains a significant global health concern. Although the incidence has declined in developed nations, it continues to pose challenges in developing countries. This article provides an updated approach to the diagnosis and management of enteric fever.

Diagnosis

The gold standard for diagnosing enteric fever is blood culture, which has a sensitivity of 40-60%. Bone marrow culture increases sensitivity to 80%, but its invasive nature limits its use. Widal test, though widely used, has poor sensitivity and specificity. Recent advances include PCR-based tests and the Typhidot test, which are promising but require further validation.

Management

Antibiotic therapy remains the mainstay of treatment. The choice of antibiotics has evolved due to increasing antimicrobial resistance. Fluoroquinolones, once first-line, are now reserved for severe cases due to widespread resistance. Current first-line agents include ceftriaxone and azithromycin. Vaccination is crucial for prevention, with two vaccines currently available: Vi polysaccharide vaccine and Ty21a vaccine.

Antimicrobial Resistance

A growing concern in enteric fever management is antimicrobial resistance, particularly multi-drug resistant (MDR) and extensively drug-resistant (XDR) strains. MDR strains are resistant to traditional first-line drugs (chloramphenicol, ampicillin, and trimethoprim-sulfamethoxazole), while XDR strains are resistant to fluoroquinolones and third-generation cephalosporins. These trends necessitate ongoing surveillance and stewardship efforts.

Conclusion

In conclusion, the approach to diagnosing and managing enteric fever has evolved with advances in diagnostic methods and shifts in antimicrobial resistance patterns. Clinicians must stay abreast of these changes to provide effective patient care. Future research should focus on improving diagnostic accuracy and developing new therapeutic strategies to combat resistant strains.
