Enteric fever, primarily caused by Salmonella Typhi and Paratyphi, is a systemic infection with significant global health implications. Early diagnosis and appropriate management are crucial to mitigate its morbidity and mortality.
Diagnosis of enteric fever is primarily based on clinical presentation and epidemiology. However, definitive diagnosis requires culture isolation of the causative organism, which can be challenging. Recent advances include nucleic acid amplification tests and serological tests like Typhidot and Tubex TF, which offer more rapid results.
Treatment of enteric fever involves antimicrobial therapy, supportive care, and patient education. Empirical therapy with ceftriaxone or azithromycin is recommended due to increasing antimicrobial resistance. Adjustments should be made based on culture and sensitivity results. Supportive care includes hydration and fever management.
Preventive measures include good sanitation, safe food and water practices, and vaccination. Typhoid vaccines (Vi polysaccharide and conjugate vaccines) have shown efficacy in endemic regions. Education about hygiene practices is also vital in controlling the spread of enteric fever.
In conclusion, the management of enteric fever requires a comprehensive approach involving early diagnosis, appropriate treatment, and preventive measures. As healthcare professionals, staying updated on the latest diagnostic tools and treatment guidelines is essential to provide optimal care to patients.
