Gastrointestinal bleeding (GIB) poses significant diagnostic and therapeutic challenges to healthcare professionals. Understanding the diverse etiologies and the complex management strategies is crucial for optimal patient care.
Initial evaluation of GIB involves a thorough history and physical examination. Laboratory tests, including complete blood count, coagulation profile, and liver function tests, provide essential information about the severity and potential causes of bleeding. Endoscopy remains the gold standard diagnostic tool for most causes of GIB. Upper endoscopy is typically performed for upper GIB, while colonoscopy is the first-line investigation for lower GIB. Capsule endoscopy and balloon-assisted enteroscopy are valuable for diagnosing small bowel bleeding.
Treatment of GIB should be individualized, depending on the cause, location, and severity of bleeding. Hemodynamic stabilization is the primary goal, often necessitating intravenous fluids and blood transfusions. Endoscopic therapy, including injection, thermal coagulation, and mechanical methods, is the mainstay of treatment for most causes of GIB. Non-variceal bleeding may require proton pump inhibitor therapy, while variceal bleeding often necessitates vasoactive drugs and band ligation. Surgery or interventional radiology may be required for refractory cases.
Risk stratification tools, such as the Glasgow-Blatchford and Rockall scores, help predict the need for intervention and the risk of rebleeding or mortality in GIB. These tools guide decision-making regarding hospital admission, level of care, and timing of endoscopy.
Managing GIB requires a systematic approach, encompassing prompt diagnosis, appropriate treatment, and accurate risk stratification. Continuous advancements in endoscopic techniques and therapeutic strategies promise to further improve outcomes in GIB.
