It provides an epidemiological overview, explores mechanistic underpinnings, identifies risk factors, outlines clinical features, and reviews best practices in diagnosis, management, and prevention. Recent advances and guideline recommendations are discussed, with a focus on practical strategies for healthcare professionals to optimize oral health outcomes across diverse populations.
Oral health extends beyond the absence of disease, encompassing the well-being of the oral cavity and its capacity to support functions such as eating, speaking, and social interaction. As the mouth serves as a gateway to systemic health, the preservation of oral health throughout the lifespan is a priority for both dental and medical professionals. This article reviews contemporary clinical guidance on oral health preservation, integrating recent research, mechanism-based insights, and practical recommendations relevant to multidisciplinary care teams.
Oral diseases are among the most prevalent non-communicable conditions globally, with dental caries, periodontal disease, and oral cancers comprising the bulk of the burden. The World Health Organization estimates that nearly 3.5 billion people are affected by oral diseases, with untreated dental caries being the most common health condition. The prevalence of periodontal disease rises with age, affecting more than 50% of adults over 30, while edentulism remains a significant concern in the elderly. Socioeconomic disparities contribute to uneven disease distribution, highlighting the need for targeted preventive strategies.
The pathogenesis of oral diseases involves complex interactions among microbiota, host immune responses, and environmental exposures. Salivary diagnostics, molecular assays, and imaging modalities (e.g., cone-beam computed tomography for complex pathology) enhance diagnostic precision. Routine oral cancer screening and assessment for systemic manifestations of oral disease (such as oral manifestations in diabetes or immunodeficiencies) are recommended in primary care and dental settings.
Management strategies are tailored to disease stage, patient risk profile, and comorbidities. Preventive approaches include professional cleaning, topical fluoride application, sealants, and patient education on oral hygiene and dietary modification. Restorative procedures address carious lesions, while periodontal therapy ranges from scaling and root planing to surgical intervention. Pharmacologic adjuncts such as chlorhexidine rinses, local antimicrobials, and salivary stimulants may be indicated. Multidisciplinary collaboration is critical, particularly in patients with complex medical histories or those undergoing therapies with oral side effects (e.g., oncology, transplant).

Lifespan oral health preservation requires a holistic, evidence-based approach integrating prevention, early detection, and multidisciplinary management. Advances in diagnostics, therapeutics, and public health strategies are enhancing the capacity of healthcare professionals to mitigate oral disease burden and improve systemic health outcomes. Continued research, education, and policy initiatives are essential to sustain progress and address emerging challenges in oral healthcare delivery.
