Osteoarthritis (OA) is the most prevalent form of arthritis, presenting significant challenges for patients and healthcare systems worldwide. Effective management requires a nuanced understanding of its complex pathophysiology, risk stratification, and individualized therapeutic strategies. Recent advances in regenerative medicine, including platelet-rich plasma and stem cell-based interventions, offer new hope alongside established conservative modalities. This comprehensive review synthesizes current evidence and guidelines, providing clinicians with a detailed, mechanism-driven overview of OA management from conservative care to cutting-edge regenerative therapies.

Introduction

Osteoarthritis is a chronic, progressive joint disorder characterized by cartilage degradation, subchondral bone remodeling, and synovial inflammation. Affecting millions globally, OA is a leading cause of pain and disability, particularly among older adults. The multifactorial etiology and heterogeneous clinical presentation underscore the importance of evidence-based, patient-centered management. This article critically examines the epidemiology, pathophysiology, risk factors, clinical features, diagnostic criteria, and a full spectrum of management options, with a dedicated focus on recent advances and clinical guidelines.

Epidemiology / Disease Burden

OA affects over 300 million individuals worldwide, with prevalence increasing in parallel with population aging and rising obesity rates. The knee, hip, and hand are most commonly involved, with the knee accounting for the majority of disability-adjusted life years (DALYs) attributed to OA. The socioeconomic burden is substantial, driven by pain, functional impairment, loss of productivity, and escalating healthcare costs. Epidemiological studies reveal higher prevalence in women, particularly post-menopause, and in individuals with a history of joint injury or repetitive joint loading.

Pathophysiology

OA is no longer considered solely a \"wear and tear\" disease. The pathogenesis involves a complex interplay of mechanical, biochemical, and inflammatory mechanisms. Central to OA is the degeneration of articular cartilage, mediated by matrix metalloproteinases (MMPs) and aggrecanases stimulated by pro-inflammatory cytokines (e.g., IL-1β, TNF-α). Subchondral bone sclerosis, osteophyte formation, and synovial inflammation contribute to disease progression and symptomatology. Recent research implicates the innate immune system and low-grade systemic inflammation, highlighting potential therapeutic targets beyond symptom control.

Risk Factors

Risk factors for OA are multifactorial, encompassing modifiable and non-modifiable elements. Age remains the strongest risk factor, with prevalence rising sharply after age 50. Obesity is a major modifiable risk, increasing mechanical load and systemic inflammation. Joint malalignment, previous joint injury (especially anterior cruciate ligament tears), repetitive occupational or sports-related stress, female sex, and genetic predisposition also play significant roles. Identifying and mitigating modifiable risks is essential in both primary and secondary prevention strategies.

Clinical Features

OA typically presents with joint pain exacerbated by activity and relieved by rest, morning stiffness lasting less than 30 minutes, and functional limitations. Physical examination may reveal joint tenderness, crepitus, decreased range of motion, bony enlargements (Heberden’s and Bouchard’s nodes in hand OA), and, in advanced cases, deformities. Synovial effusion may be present, but marked inflammation or erythema suggests alternative or concomitant diagnoses. The insidious onset and variable progression necessitate longitudinal assessment for optimal management.

Diagnosis

The diagnosis of OA is primarily clinical, supported by characteristic symptoms and physical findings. Radiographic evaluation remains the cornerstone for confirmation, with typical features including joint space narrowing, osteophyte formation, subchondral sclerosis, and cysts. MRI can detect early cartilage and soft tissue changes but is not routinely required. Laboratory tests are generally normal; their main role is exclusion of inflammatory arthropathies. Diagnostic criteria from organizations such as the American College of Rheumatology (ACR) aid in standardization and research.

Treatment & Management

OA management is multimodal, tailored to disease severity, joint involvement, comorbidities, and patient preference. First-line non-pharmacologic interventions include patient education, structured exercise (aerobic and strengthening), weight reduction, and physical therapy. Pharmacologic options focus on symptom relief: topical NSAIDs are recommended for hand and knee OA, while oral NSAIDs may be considered for more severe or refractory symptoms, balancing efficacy with gastrointestinal and cardiovascular risks. Intra-articular corticosteroid injections provide short-term relief for acute exacerbations. Adjunctive therapies include bracing, assistive devices, and occupational therapy to optimize function. Surgical intervention, including joint replacement, is reserved for advanced, refractory disease with significant quality-of-life impairment.

Recent Advances / Emerging Therapies

Regenerative medicine represents a paradigm shift in OA management. Platelet-rich plasma (PRP) injections, rich in growth factors, have shown promise in reducing pain and improving function, particularly in early knee OA. Mesenchymal stem cell (MSC) therapies, derived from bone marrow or adipose tissue, are under active investigation for their potential to modulate inflammation and promote cartilage repair. Despite encouraging early results, heterogeneity in preparation, dosing, and administration protocols remains a barrier to widespread adoption. Other emerging approaches include gene therapy, exosome-based treatments, and disease-modifying OA drugs (DMOADs) targeting specific molecular pathways. Rigorous randomized controlled trials are underway to define safety, efficacy, and optimal patient selection.

Guideline Recommendations

International guidelines—such as those from the Osteoarthritis Research Society International (OARSI), ACR, and European League Against Rheumatism (EULAR)—emphasize a stepwise, individualized approach to OA management. Non-pharmacologic interventions are universally recommended as the foundation of care. Pharmacologic therapies should be judiciously selected, prioritizing safety and patient comorbidities. Intra-articular injections are indicated for focal symptom exacerbations. Surgical interventions are considered when conservative measures fail. While regenerative therapies are gaining attention, most guidelines currently recommend their use within clinical trials or specialized centers until robust evidence supports routine clinical use.

Osteoarthritis management has evolved from purely symptomatic relief to an integrative, evidence-based approach addressing underlying mechanisms and patient-specific factors. Conservative measures remain the mainstay, but regenerative medicine offers exciting prospects for disease modification. Clinicians must stay abreast of emerging evidence and evolving guidelines to provide optimal, personalized care. Future research should aim to refine regenerative protocols, elucidate long-term outcomes, and identify biomarkers for tailored therapy, ultimately improving quality of life for those affected by this pervasive disease.
