Acute chest pain is a common symptom that presents a diagnostic challenge due to its varied etiology, ranging from benign to life-threatening conditions. This article aims to provide a systematic approach to evaluate and manage patients presenting with acute chest pain.
The first step in managing acute chest pain is a rapid and thorough assessment. This includes a focused history, physical examination, and initial diagnostic tests such as electrocardiogram (ECG) and chest X-ray. The aim is to identify life-threatening conditions such as acute coronary syndrome (ACS), pulmonary embolism, and aortic dissection that require immediate intervention.
Following the initial assessment, further investigations are guided by the patient's risk stratification. High-risk patients with persistent chest pain, hemodynamic instability, or ECG changes suggestive of ischemia should undergo immediate invasive strategies. In contrast, low-risk patients can be evaluated with non-invasive tests such as stress testing or coronary CT angiography.
The treatment of acute chest pain is guided by the underlying cause. For ACS, immediate reperfusion therapy is the mainstay of treatment. On the other hand, non-cardiac chest pain may be managed conservatively with analgesics and follow-up.
Regardless of the cause, all patients with acute chest pain should receive lifestyle advice and risk factor modification to prevent future cardiovascular events. This includes smoking cessation, blood pressure control, lipid-lowering therapy, and regular exercise.
Acute chest pain is a clinical challenge that requires a systematic approach to ensure timely diagnosis and treatment. By understanding the initial assessment, diagnostic approach, treatment, and prevention strategies, healthcare professionals can provide optimal care to these patients.
