The prevalence of multimorbidity in clinical practice is rising, driven by aging populations and improved survival rates for chronic diseases. Managing patients with multimorbidity is uniquely complex due to polypharmacy, overlapping symptoms, and the potential for conflicting management plans. Care navigation has emerged as a response to these complexities, aiming to streamline care, minimize fragmentation, and enhance outcomes through coordinated, patient-centered strategies. This article provides clinicians a comprehensive, evidence-based overview of care navigation approaches for multimorbidity, highlighting recent advances and key considerations in modern practice.
Multimorbidity affects approximately one in four adults globally, with prevalence exceeding 65% in populations over 65 years of age. The burden is particularly high in socioeconomically disadvantaged groups and those with limited healthcare access. Management of multimorbidity necessitates an individualized, patient-centered approach, with care navigation as a central pillar. Key components include comprehensive medication review, deprescribing where appropriate, alignment of care plans with patient preferences, and coordination among primary and specialty care providers. Multidisciplinary teams, case management, and the use of digital health platforms are increasingly employed to support navigation. Interventions such as structured care pathways, shared decision-making, and patient education are vital for optimizing outcomes and minimizing treatment burden.
Recent advances in care navigation for multimorbidity include the development of integrated care models, digital navigation tools, and artificial intelligence-driven decision support systems.
