Coronary vasomotor function plays a critical role in cardiovascular health, with growing evidence highlighting unique pathophysiological patterns in women, especially across various hormonal transitions such as menarche, pregnancy, perimenopause, and menopause. This review synthesizes current knowledge on the epidemiology, mechanisms, clinical features, diagnostic approaches, management strategies, and emerging therapies relevant to coronary vasomotor dysfunction in women. Emphasis is placed on the clinical implications of hormonal influences, contemporary guideline recommendations, and future directions in research and practice.
Sex-specific differences in cardiovascular disease (CVD) presentation and outcomes are increasingly recognized as pivotal determinants of women’s health. Coronary vasomotor dysfunction, encompassing both epicardial and microvascular abnormalities, is particularly prevalent among women and is often unmasked or exacerbated during periods of hormonal fluctuation. Understanding the interplay between hormonal transitions and coronary vasomotor function is imperative for optimizing diagnosis and management, as conventional paradigms may under-recognize or inadequately address these complex pathologies in women.
Coronary vascular dysfunction is estimated to affect up to 40% of women presenting with angina and non-obstructive coronary arteries, especially during the perimenopausal and menopausal periods. Epidemiological data from large cohort studies such as WISE (Women’s Ischemia Syndrome Evaluation) demonstrate that women are disproportionately burdened by microvascular angina compared to men. The prevalence of vasomotor dysfunction rises with advancing age and is notably higher during menopausal transitions, contributing significantly to the overall burden of ischemic heart disease in women globally.
Estrogens exert profound vasoprotective effects through genomic and non-genomic pathways, modulating endothelial nitric oxide synthesis, vascular smooth muscle tone, and inflammatory responses. During hormonal transitions such as menopause, declining estrogen levels result in impaired endothelial function, increased sympathetic activity, and heightened vascular reactivity. These changes culminate in a predisposition to both epicardial spasm and microvascular dysfunction. Progesterone and androgens also modulate vascular tone, but their net impact during transitions remains less well-defined. The interplay between hormonal milieu, oxidative stress, and systemic inflammation represents a central mechanistic axis in the pathophysiology of coronary vasomotor dysfunction in women.
Key risk factors for coronary vasomotor dysfunction in women include age, early menopause, polycystic ovary syndrome (PCOS), history of gestational complications (e.g., preeclampsia), metabolic syndrome, and autoimmune diseases. Lifestyle factors such as smoking, physical inactivity, and psychosocial stress further compound risk. Genetic predispositions, including polymorphisms in estrogen receptor and nitric oxide synthase genes, may modulate individual susceptibility. Notably, traditional cardiovascular risk factors often underestimate risk in women with predominant vasomotor dysfunction, underscoring the need for sex-specific risk stratification tools.
Women with coronary vasomotor dysfunction frequently present with atypical or non-exertional chest pain, dyspnea, palpitations, or unexplained fatigue. Angina may occur at rest or be triggered by emotional stress, and is often refractory to conventional anti-anginal therapies. Clinical suspicion should be heightened in women with recurrent symptoms and non-obstructive coronary arteries on angiography, particularly during periods of hormonal transition. Comorbid conditions such as migraine, Raynaud’s phenomenon, and irritable bowel syndrome are common and may reflect shared vascular dysregulation.
Diagnosis of coronary vasomotor dysfunction in women relies on a combination of clinical assessment, non-invasive testing, and invasive coronary function testing. Non-invasive modalities include stress echocardiography, cardiac MRI with perfusion imaging, and positron emission tomography (PET) to assess microvascular function. Invasive testing with acetylcholine or adenosine provocation during coronary angiography remains the gold standard for definitive diagnosis, allowing differentiation between epicardial spasm, microvascular angina, and mixed phenotypes. Emerging biomarkers such as circulating endothelial progenitor cells and microRNAs are under investigation for their diagnostic potential.
Management of coronary vasomotor dysfunction in women requires an individualized, multifaceted approach. Lifestyle modification, including smoking cessation, exercise, and stress reduction, forms the cornerstone of therapy. Pharmacological interventions encompass nitrates, calcium channel blockers, ACE inhibitors, and statins, with beta-blockers reserved for select cases. In women with refractory symptoms, novel agents such as ranolazine or low-dose tricyclic antidepressants may be considered. Hormone replacement therapy (HRT) remains controversial, with current evidence not supporting its routine use solely for cardiovascular protection. Patient education and multidisciplinary care, including psychological support, are essential for optimizing outcomes.
Recent advances in the understanding of endothelial biology and vasomotor regulation have spurred the development of targeted therapies. Rho-kinase inhibitors, endothelin receptor antagonists, and novel anti-inflammatory agents are under investigation for their potential to modulate coronary vasomotor tone. Precision medicine approaches, incorporating genetic and proteomic profiling, promise to refine risk stratification and guide therapy. Additionally, non-pharmacologic interventions such as mindfulness-based stress reduction and cardiac rehabilitation are gaining traction for their beneficial effects on vascular function and quality of life.
Contemporary guidelines from the American Heart Association (AHA) and European Society of Cardiology (ESC) advocate for heightened clinical awareness of coronary vasomotor dysfunction in women, particularly across hormonal transitions. Recommendations emphasize the importance of comprehensive cardiovascular risk assessment, appropriate use of diagnostic testing, and individualized management strategies. The guidelines also call for increased research focused on sex-specific mechanisms and therapeutic targets, as well as the integration of women into clinical trials addressing coronary vascular disease.
Coronary vasomotor dysfunction represents a clinically significant and under-recognized contributor to cardiovascular morbidity in women, particularly during hormonal transitions. Advances in understanding the unique pathophysiology, risk factors, and clinical presentations have informed more nuanced diagnostic and therapeutic strategies. Ongoing research and evolving guidelines are expected to further enhance the care of women with coronary vasomotor dysfunction, ultimately reducing the burden of ischemic heart disease in this population.
