Vertigo, a common clinical syndrome characterized by the sensation of spinning or a feeling of movement when stationary, is a complex condition that often challenges clinicians. A comprehensive approach is essential for accurate diagnosis and effective management.
Vertigo, often associated with vestibular disorders, is not a disease itself but a symptom of various underlying conditions. It is crucial to differentiate it from non-vestibular dizziness for appropriate treatment.
A detailed patient history is the cornerstone of vertigo diagnosis. The onset, duration, triggers, and associated symptoms can provide valuable insights. Physical examination, including bedside vestibular tests, can further aid in localizing the lesion. Investigations such as audiometry, vestibular evoked myogenic potentials (VEMP), and imaging may be required based on clinical suspicion.
Management of vertigo is tailored based on the underlying cause. Acute vertigo due to vestibular neuritis can be managed with vestibular suppressants, while benign paroxysmal positional vertigo (BPPV) can be treated with repositioning maneuvers. Chronic vertigo conditions like Ménière's disease may require long-term strategies including dietary modifications, diuretics, and vestibular rehabilitation.
Vestibular rehabilitation, an exercise-based program, is an integral part of vertigo management. It aims to promote central nervous system compensation for inner ear deficits. It includes habituation exercises, gaze stabilization exercises, and balance training.
Vertigo is a multifaceted symptom requiring a comprehensive clinical approach for successful diagnosis and management. A thorough understanding of the underlying mechanisms, a systematic diagnostic approach, and a patient-specific management plan are key to improving patient outcomes and quality of life.
