Microglial lipid metabolism has emerged as a pivotal area of neuroscience, linking cellular energetics, immunologic function, and neuropathology. Recent insights underscore the role of lipid processing in microglia for brain development, homeostasis, and disease progression, influencing neuroinflammatory responses and neurodegenerative pathologies. This review summarizes current understanding of microglial lipid metabolism, its mechanistic underpinnings, epidemiological relevance, clinical implications, diagnostic challenges, therapeutic opportunities, recent advances, and guideline-based recommendations for clinical practice.
Microglia, the resident immune cells of the central nervous system (CNS), orchestrate critical functions in health and disease. Beyond their roles in immune surveillance and neuroinflammation, microglia exhibit complex metabolic phenotypes tailored to their functional states. Among various metabolic pathways, lipid metabolism stands out for its influence on microglial activation, phagocytosis, and interaction with other CNS cell types. Dysregulation of lipid processing in microglia is increasingly associated with neurodegenerative disorders, including Alzheimer’s disease (AD), Parkinson’s disease (PD), and multiple sclerosis (MS). Understanding these mechanisms is essential for clinicians and researchers aiming to develop targeted therapeutic strategies.
Global epidemiological data indicate that disorders linked to microglial dysfunction, particularly neurodegenerative diseases, are on the rise due to an aging population. Alzheimer’s disease, affecting over 55 million individuals worldwide, is a primary example where microglial lipid metabolism is implicated. The prevalence of Parkinson’s and various leukodystrophies also illustrates the broad disease burden related to microglial metabolic dysregulation. Inflammatory and demyelinating diseases such as MS further contribute to substantial morbidity, with microglial lipid processing being central to disease propagation and progression.
Microglial lipid metabolism encompasses uptake, storage, and catabolism of diverse lipid classes, including cholesterol, sphingolipids, and phospholipids. Key molecular players include apolipoprotein E (ApoE), triggering receptor expressed on myeloid cells 2 (TREM2), and peroxisome proliferator-activated receptors (PPARs). TREM2 mutations, for instance, impair lipid sensing and phagocytosis, predisposing individuals to AD and frontotemporal dementia. Lipid droplets within microglia, once considered inert, are now recognized as dynamic organelles influencing inflammatory signaling and oxidative stress. Aberrant lipid metabolism leads to accumulation of toxic lipid species, mitochondrial dysfunction, and chronic neuroinflammation, driving neuronal injury and synaptic deficits.
Genetic mutations in genes regulating lipid metabolism—such as APOE4, TREM2, and GBA—confer increased risk for neurodegenerative diseases. Environmental factors, including high-fat diets and metabolic syndrome, also impact microglial lipid handling. Aging is a non-modifiable risk factor, associated with altered lipid profiles and impaired microglial function. Chronic inflammation, traumatic brain injury, and exposure to neurotoxins further exacerbate lipid metabolic dysfunction, potentiating disease onset and progression.
Clinical manifestations of disorders involving microglial lipid metabolism are heterogeneous, ranging from cognitive decline, memory impairment, and motor dysfunction to psychiatric disturbances. In AD, early symptoms include progressive memory loss and executive dysfunction, while PD presents with bradykinesia, rigidity, and tremors. Leukodystrophies and lysosomal storage diseases often manifest in childhood with motor regression, seizures, and developmental delay. Importantly, microglial lipid pathology may precede overt clinical symptoms, highlighting the need for early biomarkers.
Diagnosis of microglial lipid metabolic disorders relies on a combination of clinical assessment, neuroimaging, and molecular biomarkers. Advanced MRI techniques can detect white matter changes and neuroinflammation. CSF and plasma markers, such as soluble TREM2, ApoE isoforms, and specific lipid metabolites, are under investigation for early detection and disease monitoring. Genetic testing for APOE, TREM2, and other mutations informs prognosis and therapeutic decision-making. Functional assays assessing microglial phagocytic capacity and lipid storage are emerging research tools.
Currently, treatment strategies are largely symptomatic, focusing on neuroprotection and management of comorbidities. Cholinesterase inhibitors and NMDA receptor antagonists are used in AD, while dopamine replacement therapy is standard in PD. Disease-modifying therapies targeting microglial activation and lipid metabolism are under active investigation. Lifestyle interventions promoting cardiovascular and metabolic health may indirectly benefit microglial function. Multidisciplinary care, including cognitive rehabilitation and psychosocial support, is essential for optimal patient outcomes.
Recent research has illuminated new therapeutic avenues by targeting microglial lipid metabolism. Agonists of PPARs and liver X receptors (LXRs) show promise in modulating lipid handling and reducing neuroinflammation. Small molecules and antibodies targeting TREM2 and ApoE pathways are in preclinical and early clinical trials. Lipid nanoparticle-based drug delivery systems are being explored for selective modulation of microglial metabolism. Advances in single-cell transcriptomics and lipidomics are refining our understanding of microglial heterogeneity and identifying subpopulations amenable to targeted intervention.
Current consensus guidelines emphasize the importance of early recognition of neurodegenerative disorders and risk stratification using genetic and metabolic markers. The integration of metabolic screening in high-risk populations is recommended. While disease-specific guidelines for microglial lipid metabolism are evolving, clinicians are encouraged to adopt a holistic approach encompassing lifestyle modification, management of systemic metabolic risk factors, and participation in clinical trials investigating targeted therapies. Ongoing education and interdisciplinary collaboration are essential as emerging evidence reshapes clinical paradigms.
Microglial lipid metabolism is a critical determinant of CNS health, linking genetic risk, environmental exposures, and disease pathogenesis. Advances in mechanistic understanding are translating into novel diagnostic and therapeutic strategies. For clinicians, integrating knowledge of microglial lipid biology into practice offers the prospect of improved patient outcomes in neurodegenerative and neuroinflammatory disorders. Continued research and guideline development will refine management approaches and drive innovation in this rapidly evolving field.
