Enteric fever, primarily caused by Salmonella Typhi and Salmonella Paratyphi, is a significant global health concern. This systemic infection, prevalent in regions with poor sanitation and water hygiene, requires adept clinical acumen for diagnosis and management. This article provides a comprehensive guide to the treatment of enteric fever for healthcare professionals.
Diagnosis is primarily based on clinical presentation, with fever, abdominal pain, and constipation being common symptoms. Blood culture remains the gold standard for diagnosis, although it has a sensitivity of only 40-60%. Molecular methods, such as PCR, are increasingly used for their higher sensitivity and specificity.
The cornerstone of enteric fever management is antimicrobial therapy. The choice of antibiotic depends on local susceptibility patterns. Fluoroquinolones were once the drug of choice but increasing resistance has led to the use of third-generation cephalosporins and azithromycin. Empirical therapy should be adjusted based on culture and sensitivity results.
Complications such as intestinal perforation and haemorrhage require immediate surgical intervention. Other complications like myocarditis, hepatitis, and neuropsychiatric manifestations necessitate supportive care alongside antimicrobial therapy.
Prevention strategies include improving sanitation, safe water supplies, and food safety. Vaccination also plays a crucial role in prevention, with two vaccines currently available: the oral live-attenuated Ty21a vaccine and the injectable Vi polysaccharide vaccine.
Enteric fever remains a global health challenge, particularly in low-resource settings. Successful management requires a multifaceted approach, encompassing accurate diagnosis, effective antimicrobial therapy, management of complications, and prevention strategies. As healthcare professionals, staying abreast of the latest developments in enteric fever management is crucial to providing optimal patient care.
