Fever results from an elevation in set-point temperature regulated by the hypothalamus, often triggered by pyrogens. These pyrogens can be either exogenous, such as bacteria and viruses, or endogenous, like interleukin-1 (IL-1) and tumor necrosis factor (TNF). Recognizing the source is essential for appropriate treatment.
Initial evaluation should focus on history and physical examination to identify potential sources of infection or non-infectious causes. Laboratory tests, including complete blood count, blood cultures, and inflammatory markers, can provide additional information. Imaging studies may be warranted based on clinical suspicion.
Treatment should aim at the underlying cause. Antipyretics like acetaminophen and nonsteroidal anti-inflammatory drugs (NSAIDs) can be used to alleviate discomfort. However, they should not replace the definitive treatment of the primary disease. In cases of severe infection, prompt initiation of empirical antibiotics is critical, pending culture results.
Patient monitoring is vital to assess the response to treatment and detect any potential complications. Regular follow-up visits should be scheduled to ensure resolution of the fever and address any lingering issues.
Managing fever in clinical practice requires a thorough understanding of its pathophysiology, careful evaluation, and targeted treatment. Regular monitoring and follow-up are also essential to ensure favorable patient outcomes. By following these guidelines, clinicians can effectively manage fevers and improve patient care.
