Introduction

As the second most common neurodegenerative disorder globally, Parkinson's Disease (PD) presents a significant challenge in modern healthcare. Understanding its pathophysiology and management is crucial for clinicians to provide optimal care.

Pathophysiology of Parkinson's Disease

PD is characterized by the progressive loss of dopaminergic neurons in the substantia nigra, leading to a reduction in striatal dopamine. This imbalance disrupts the basal ganglia circuits, resulting in the classic motor symptoms of PD: bradykinesia, resting tremor, rigidity, and postural instability. Non-motor symptoms include autonomic dysfunction, cognitive impairment, and mood disorders, which are attributed to the involvement of non-dopaminergic systems.

Diagnosis and Clinical Features

Diagnosis of PD is primarily clinical, based on the presence of cardinal motor symptoms and response to dopaminergic therapy. Neuroimaging techniques, like DaTscan, can support the diagnosis, but are not routinely used due to their cost and limited availability. The non-motor symptoms often precede the motor signs, providing a potential window for early diagnosis and intervention.

Management of Parkinson's Disease

Management of PD is multidisciplinary, involving pharmacological and non-pharmacological approaches. Pharmacological therapy is centered on restoring dopamine levels using levodopa, dopamine agonists, and MAO-B inhibitors. Deep Brain Stimulation (DBS) is an option for patients with advanced disease and motor complications. Non-pharmacological interventions include physiotherapy, occupational therapy, and speech therapy, which address the motor and non-motor symptoms, improving the quality of life.

Conclusion

PD is a complex neurodegenerative disorder with a multifaceted pathophysiology. While our understanding of the disease has significantly improved, challenges remain in early diagnosis and management. Future research should focus on identifying early biomarkers and developing novel therapeutic strategies, aiming to slow or halt disease progression and improve patient outcomes.
