Introduction

Chronic medical conditions, including but not limited to diabetes mellitus, chronic obstructive pulmonary disease (COPD), heart failure, and chronic kidney disease, are prevalent and persistent contributors to global morbidity. Functional decline, defined as a progressive loss of physical, cognitive, or social abilities, is a multifactorial consequence affecting a substantial proportion of these patients. Understanding the mechanisms, risk factors, and evidence-based strategies to prevent or attenuate functional decline is essential for clinicians managing such patients. Comprehensive preventive interventions can delay disability, reduce healthcare costs, and improve longevity and quality of life.

Epidemiology / Disease Burden

Comprehensive geriatric assessment (CGA) integrates physical, cognitive, psychological, and social domains to provide a holistic understanding of patient needs. Biomarkers, imaging, and disease-specific assessments further elucidate the extent and contributors to functional impairment.

Treatment & Management

Preventing functional decline requires a multifaceted, individualized approach. Key interventions include optimizing disease-specific management (e.g., glycemic control in diabetes, bronchodilator therapy in COPD), promoting physical activity and structured exercise programs, nutritional support, medication review to limit polypharmacy, and addressing social determinants of health. Multidisciplinary rehabilitation, including physiotherapy and occupational therapy, is fundamental. Early mobilization during hospital admissions and transitional care planning can mitigate in-hospital and post-discharge functional loss.

Recent Advances / Emerging Therapies

Recent research highlights the potential of targeted therapies such as myostatin inhibitors and SGLT2 inhibitors to improve muscle mass and physical performance in select populations. Digital health technologies, including tele-rehabilitation and wearable activity monitors, are being integrated into care pathways to enhance patient engagement and real-time monitoring. Guideline Recommendations

Professional societies, including the American Heart Association, American Diabetes Association, and European Respiratory Society, recommend routine assessment of functional status and early implementation of preventive strategies. Guidelines endorse structured exercise programs, nutritional optimization, and psychosocial support as standard care components. They also emphasize the importance of shared decision-making, regular medication reviews, and interdisciplinary team management to address the complex needs of patients at risk for functional decline.
