Multimorbidity, the coexistence of two or more chronic medical conditions in an individual, represents a complex and rapidly growing health challenge worldwide. Emerging research highlights the pivotal role of immune resilience—the immune system’s capacity to adapt and respond effectively to diverse stressors—in influencing disease trajectories and outcomes in multimorbid populations. This review synthesizes current scientific understanding of immune resilience within the context of multimorbidity, elucidates underlying mechanisms, discusses clinical implications, and examines recent advances and guideline-based strategies for optimizing patient care.
Multimorbidity is increasingly prevalent, particularly in aging populations, posing significant challenges for healthcare systems and professionals. The intricate interplay of multiple chronic diseases, such as diabetes, cardiovascular disease, and chronic kidney disease, often leads to compounded physiological stress and immune dysfunction. Immune resilience has emerged as a key determinant in modulating disease progression and patient outcomes, warranting comprehensive exploration of its mechanisms and clinical relevance. Understanding and enhancing immune resilience could transform management paradigms for patients with complex multimorbid profiles.
Globally, the prevalence of multimorbidity ranges from 20% to over 60% in older adults, with substantial variation based on geographic, socioeconomic, and demographic factors. The presence of multiple chronic diseases is associated with increased health care utilization, polypharmacy, reduced quality of life, and elevated mortality risk. Notably, the burden of multimorbidity extends beyond individual disease management, necessitating integrated approaches that address the cumulative impact on immune function and systemic health. Recent epidemiological studies underscore the disproportionate effect on underserved populations, further amplifying health disparities.
The pathophysiology of multimorbidity is characterized by persistent low-grade inflammation, immune senescence, and dysregulation of innate and adaptive immune responses. Chronic diseases such as obesity, type 2 diabetes, and hypertension contribute to a pro-inflammatory milieu through mechanisms including increased cytokine production, oxidative stress, and endothelial dysfunction. Immune resilience is compromised as regulatory pathways become impaired, leading to heightened susceptibility to infections, delayed tissue repair, and suboptimal vaccine responses. Central to these processes is the disruption of immunometabolic homeostasis, which exacerbates organ dysfunction and accelerates biological aging.
Key risk factors for reduced immune resilience in multimorbidity include advanced age, genetic predisposition, persistent stress, poor nutritional status, sedentary lifestyle, and environmental exposures such as pollution or chronic infections. Polypharmacy and inappropriate medication use may further impair immune function through drug-drug interactions and adverse effects. Socioeconomic determinants, such as limited access to healthcare, social isolation, and psychological distress, are increasingly recognized as modulators of both multimorbidity risk and immune competence.
Clinically, patients with compromised immune resilience in the context of multimorbidity often present with increased frequency and severity of infections, delayed wound healing, atypical disease presentations, and poor responses to standard therapies. These individuals may exhibit signs of frailty, cognitive impairment, and diminished functional reserve. The heterogeneity of multimorbid profiles necessitates individualized assessment, with attention to subtle changes in clinical status that may signal immune dysregulation or decompensation.
Diagnosis of immune resilience impairment in multimorbid patients relies on a combination of clinical assessment, laboratory markers of inflammation (e.g., C-reactive protein, interleukin-6), immune cell profiling, and functional assays. Comprehensive geriatric assessment and frailty indices can provide valuable insights into physiological reserve. Recent advances in multi-omics and systems biology approaches offer the potential for personalized immune profiling, enabling risk stratification and targeted interventions.
Management of multimorbid patients with reduced immune resilience requires a holistic, multidisciplinary approach. Optimization of comorbid conditions, avoidance of polypharmacy, and judicious use of immunomodulatory agents are foundational. Nutritional support, physical activity, and psychosocial interventions have demonstrated benefits in enhancing immune competence. Vaccination strategies should be individualized, with consideration of altered vaccine responses in this population. Regular monitoring and early intervention for infections or exacerbations are critical to maintaining stability and preventing complications.
Recent research highlights several promising avenues for bolstering immune resilience in multimorbidity. Senolytic agents targeting cellular senescence, immunonutrition, gut microbiome modulation, and personalized immunotherapeutic strategies are under investigation. Advances in biomarker discovery and digital health tools enable real-time monitoring of immune status and early detection of decompensation. Furthermore, interdisciplinary care models integrating geriatric, immunologic, and primary care perspectives are demonstrating improved patient outcomes in pilot studies.
Current clinical guidelines emphasize individualized, patient-centered care for multimorbid populations. Key recommendations include regular comprehensive assessments, medication review and de-prescribing where appropriate, and implementation of integrated care pathways. Guidelines advocate for the inclusion of frailty and immune competence assessments in routine practice, as well as proactive vaccination and infection prevention strategies. Multidisciplinary collaboration and shared decision-making are essential for optimizing care and supporting immune resilience.
Immune resilience is a critical determinant of health outcomes in individuals with multimorbidity. Improved understanding of the mechanisms underlying immune dysfunction, coupled with evidence-based interventions, offers the potential to mitigate disease burden and enhance quality of life for this growing patient population. Ongoing research and innovation are essential to refine diagnostic tools, develop targeted therapies, and inform clinical guidelines, ultimately advancing the care of multimorbid patients through the lens of immune resilience.
