Enteric fever, primarily caused by Salmonella Typhi and Paratyphi, remains a significant global health concern. It is paramount for healthcare professionals to stay abreast of the latest diagnostic and treatment strategies due to the increasing antimicrobial resistance.
Traditionally, blood culture has been the gold standard for diagnosing enteric fever. However, its sensitivity varies based on the disease stage and prior antibiotic use. Newer diagnostic methods like polymerase chain reaction (PCR) and serological tests have shown promise in improving diagnostic accuracy. PCR offers rapid, highly sensitive, and specific detection of Salmonella Typhi and Paratyphi. Serological tests, such as the Typhidot test and Tubex TF test, provide results within hours, aiding in quicker patient management.
Antimicrobial resistance necessitates the continuous updating of treatment regimens. Fluoroquinolones, previously the first-line treatment, are now reserved due to increasing resistance. Current guidelines recommend empiric therapy with third-generation cephalosporins, such as ceftriaxone and cefixime, or azithromycin. For confirmed cases of multidrug-resistant strains, newer agents like meropenem and tigecycline may be used.
Vaccination plays a crucial role in enteric fever prevention. Two vaccines are currently available: an oral live-attenuated Ty21a vaccine and a Vi polysaccharide vaccine. Both vaccines provide substantial protection and are recommended for high-risk populations and travelers to endemic areas.
Advancements in diagnostic methods and evolving treatment regimens have improved the management of enteric fever. Regularly updating knowledge on these advancements is vital for healthcare professionals to provide optimal patient care. Ongoing research and surveillance are crucial to monitor disease trends and resistance patterns, ensuring effective future strategies for enteric fever control.
