Pulmonary hypertension (PH) is a complex, multifactorial disease characterized by elevated pulmonary artery pressure. This article provides an overview of the disease's pathophysiology, diagnostic approaches, and treatment options.
PH is a hemodynamic and pathophysiological condition defined by an increase in mean pulmonary arterial pressure ≥ 25 mmHg at rest. This condition can lead to right heart failure and death if left untreated. The disease's pathophysiology involves three major processes: vasoconstriction, vascular remodeling, and thrombosis.
Diagnosing PH requires a high index of suspicion. Initial diagnostic procedures include echocardiography and right heart catheterization. Echocardiography is used to estimate pulmonary artery pressure, while right heart catheterization confirms the diagnosis. Other diagnostic tools such as computed tomography (CT), magnetic resonance imaging (MRI), and ventilation-perfusion (V/Q) scan may also be used to determine the underlying cause.
Treatment of PH is tailored based on the underlying cause, disease severity, and patient's overall health. Current therapeutic strategies aim to improve symptoms, slow disease progression, and improve survival. These include general measures (oxygen therapy, diuretics), targeted PH therapies (endothelin receptor antagonists, phosphodiesterase type 5 inhibitors, prostacyclin analogs), and advanced therapies (atrial septostomy, lung transplantation).
In conclusion, PH is a complex disease requiring a comprehensive understanding of its pathophysiology, diagnosis, and treatment. Early recognition and prompt initiation of appropriate therapy can significantly improve patient outcomes. As healthcare professionals, it is our responsibility to stay abreast with the latest developments in the field to provide optimal care to our patients.
