Coronary microvascular dysfunction (CMD) and impaired endothelial health are increasingly recognized as pivotal contributors to the pathogenesis and progression of cardiovascular disease (CVD). This review synthesizes current evidence on mechanisms of coronary microvascular resilience, the importance of endothelial health preservation, and their roles in primary and secondary prevention of CVD. We discuss epidemiological trends, pathophysiological underpinnings, risk stratification, clinical features, diagnostic modalities, therapeutic strategies, recent advances, and guideline recommendations, providing clinicians with an up-to-date framework for optimizing cardiovascular prevention in diverse patient populations.
Cardiovascular morbidity and mortality remain leading global health concerns, with coronary artery disease (CAD) as a principal driver. Beyond obstructive epicardial lesions, the coronary microcirculation and the integrity of the endothelium are now understood to be central to disease development and progression. Coronary microvascular resilience the ability of the microvasculature to adapt and maintain perfusion under stress and preservation of endothelial function are fundamental to cardiovascular homeostasis. This review aims to elucidate the clinical and mechanistic significance of these factors in prevention, highlighting actionable insights for healthcare professionals.
CMD affects a substantial proportion of patients presenting with angina and non-obstructive coronary arteries, with prevalence estimates ranging from 30% to 70% in women and 20% to 50% in men with chest pain syndromes. Endothelial dysfunction, as measured by impaired flow-mediated dilation, is detectable in up to 40% of middle-aged adults and predicts adverse cardiovascular outcomes. The burden is amplified in populations with metabolic syndrome, diabetes, hypertension, and chronic inflammatory states, underscoring the need for population-level and individualized preventive strategies.
The coronary microvasculature, comprising arterioles, capillaries, and venules, modulates myocardial perfusion via dynamic vasoreactivity and structural remodeling. Endothelial cells orchestrate vascular tone, barrier function, and homeostasis through nitric oxide (NO) production, prostacyclin release, and regulation of inflammation and thrombosis. Oxidative stress, hyperglycemia, dyslipidemia, and systemic inflammation disrupt endothelial integrity, diminish NO bioavailability, and promote microvascular rarefaction and remodeling. This cascade leads to perfusion heterogeneity, ischemia, and ultimately, myocardial dysfunction even in the absence of epicardial disease.
Traditional CVD risk factors age, hypertension, diabetes mellitus, dyslipidemia, smoking, and obesity exert deleterious effects on both the endothelium and coronary microvasculature. Emerging evidence implicates psychosocial stress, sleep disorders, systemic autoimmune diseases, and exposure to air pollutants as additional contributors. Genetic predispositions affecting endothelial nitric oxide synthase (eNOS) and other signaling pathways further modulate individual susceptibility to CMD and endothelial dysfunction.
CMD often presents with exertional angina, microvascular angina (formerly syndrome X), and atypical chest pain, frequently in women and individuals with non-obstructive coronary arteries. Symptoms may be accompanied by transient ECG changes or evidence of ischemia on functional testing without overt epicardial stenosis. Systemic endothelial dysfunction can manifest as impaired peripheral vasodilation, increased vascular stiffness, and heightened thrombotic risk, informing clinical suspicion in at-risk populations.
Assessment of coronary microvascular function and endothelial health is multifaceted. Invasive techniques, such as coronary flow reserve (CFR) measurement and acetylcholine-provocation testing during cardiac catheterization, directly evaluate microvascular and endothelial responsiveness. Noninvasive options include positron emission tomography (PET), cardiac magnetic resonance imaging (CMR) with perfusion mapping, and peripheral flow-mediated dilation (FMD) via ultrasound. Biomarkers such as asymmetric dimethylarginine (ADMA), high-sensitivity C-reactive protein (hsCRP), and endothelial microparticles may offer adjunctive diagnostic information.
Primary prevention of CMD and preservation of endothelial health hinge on aggressive risk factor modification: blood pressure optimization, glycemic control, lipid lowering, smoking cessation, regular physical activity, and dietary intervention. Pharmacotherapy with statins, ACE inhibitors, angiotensin receptor blockers, and sodium-glucose cotransporter 2 (SGLT2) inhibitors has demonstrated benefits in improving endothelial function and microvascular resilience. Antiplatelet agents, vasodilators such as calcium channel blockers, and targeted anti-inflammatory therapies may provide symptomatic and prognostic benefits in select populations.
Recent research emphasizes the promise of novel agents and interventions aimed at enhancing microvascular health. Endothelin receptor antagonists, soluble guanylate cyclase stimulators, and selective NO donors are under investigation for their roles in restoring endothelial and microvascular function. Advances in regenerative therapies, including endothelial progenitor cell therapy and microRNA-based strategies, offer future avenues for reversing microvascular injury. Implementation of wearable technology and advanced imaging for real-time endothelial assessment is progressing toward clinical utility.
Contemporary guidelines from the European Society of Cardiology (ESC), American Heart Association (AHA), and American College of Cardiology (ACC) advocate for the recognition and management of CMD and endothelial dysfunction, particularly in high-risk and symptomatic patients. Multimodal risk assessment, aggressive cardiovascular risk factor control, and individualized pharmacological therapy are recommended. Lifestyle modification remains foundational, with emphasis on exercise, plant-based diets, and weight management. The integration of noninvasive diagnostic modalities and emerging biomarkers is encouraged for comprehensive risk stratification.
Coronary microvascular resilience and endothelial health preservation represent crucial, yet often underappreciated, dimensions of cardiovascular prevention. Understanding the mechanisms underlying microvascular and endothelial dysfunction provides clinicians with opportunities to intervene early, personalize risk reduction, and improve long-term outcomes. Ongoing research and innovation are poised to further elucidate therapeutic targets and refine preventive strategies, reinforcing the imperative for proactive and holistic cardiovascular care.
