Liver cirrhosis, a late-stage scarring of the liver, is a serious condition that often presents numerous complications. These complications can be life-threatening if not properly managed, necessitating a comprehensive understanding among healthcare professionals.
Complications often arise due to portal hypertension and liver insufficiency. These may include ascites, variceal bleeding, hepatic encephalopathy, and hepatorenal syndrome. Other complications such as hepatocellular carcinoma may also occur.
Ascites, the most common complication, can be managed with salt restriction, diuretics, and therapeutic paracentesis. For refractory ascites, transjugular intrahepatic portosystemic shunt (TIPS) or liver transplant may be considered.
Variceal bleeding is a major cause of death. Primary prophylaxis with non-selective beta-blockers or endoscopic variceal ligation can reduce the risk. Acute bleeding requires immediate medical attention, with vasoactive drugs, antibiotics, and endoscopic therapy as the mainstay of treatment.
Hepatic encephalopathy is managed by treating the precipitating cause and reducing gut ammonia production and absorption using lactulose or rifaximin. For persistent cases, additional therapies may be needed.
Hepatorenal syndrome is managed with vasoconstrictors and albumin. Dialysis may be required in severe cases. TIPS or liver transplant can be considered for refractory cases.
Regular screening for hepatocellular carcinoma is crucial given the increased risk in cirrhotic patients. Surveillance with ultrasound, with or without alpha-fetoprotein measurement, is recommended every six months.
Understanding and managing complications associated with liver cirrhosis is crucial for improving patient outcomes. The complexity of this disease necessitates a comprehensive, multidisciplinary approach to care, with a focus on preventative measures, early detection, and prompt treatment of complications.
