Introduction

Chronic heart disease encompasses a spectrum of disorders, including chronic heart failure, stable ischemic heart disease, and cardiomyopathies, each contributing to long-term morbidity and mortality worldwide. Beyond the physiological burden, CHD often leads to diminished self-efficacy, activity avoidance, and reduced lifestyle confidence, undermining holistic patient well-being. Restoring lifestyle confidence requires a nuanced understanding of the interplay between medical management and psychosocial interventions, guided by recent scientific advancements and evidence-based guidelines.

Epidemiology / Disease Burden

Optimal management of CHD involves a dual focus on evidence-based pharmacotherapeutics and non-pharmacological interventions. Guideline-directed medical therapy includes the use of beta-blockers, renin-angiotensin system inhibitors, mineralocorticoid receptor antagonists, sodium-glucose co-transporter 2 inhibitors, and, when indicated, device therapy such as implantable cardioverter-defibrillators or cardiac resynchronization. Structured cardiac rehabilitation programs are central to restoring lifestyle confidence, providing supervised exercise, education, psychological support, and risk factor modification. Multidisciplinary care, including input from physiotherapists, psychologists, and social workers, enhances functional recovery and promotes reintegration into daily activities.

Recommendations include routine assessment of psychosocial well-being, referral to structured cardiac rehabilitation, optimization of medical therapy, and ongoing patient education. Guideline updates increasingly recognize lifestyle confidence and patient-reported outcomes as critical measures of therapeutic success, advocating for interventions that address both physiological and psychological domains of the disease.
