Introduction

Chronic cough, defined as a cough persisting for more than eight weeks, is a common presenting complaint in primary care and pulmonology clinics. Despite its prevalence, diagnosing and managing chronic cough can be challenging due to its multifactorial etiology. This article seeks to provide a comprehensive approach to diagnosing and managing chronic cough in clinical practice.

Etiology and Diagnosis

Chronic cough can be attributed to a variety of causes, including gastroesophageal reflux disease (GERD), asthma, and upper airway cough syndrome (UACS) secondary to rhinosinus diseases. Other causes include chronic bronchitis, bronchiectasis, and non-asthmatic eosinophilic bronchitis. A thorough history and physical examination are essential for identifying potential causes. Diagnostic tests such as chest radiography, spirometry, and bronchoprovocation tests can be utilized to confirm the diagnosis.

Management Approach

The management of chronic cough should be tailored to its underlying cause. For GERD-related cough, lifestyle modifications and proton pump inhibitors are the first-line treatment. Inhaled corticosteroids are recommended for cough-variant asthma, while antihistamines and nasal corticosteroids are effective for UACS. For patients with non-asthmatic eosinophilic bronchitis, inhaled corticosteroids are beneficial. Empirical treatment trials can be considered when the diagnosis is uncertain.

Challenges and Future Directions

Despite these strategies, chronic cough remains a challenging condition to manage. Some patients continue to experience symptoms despite treatment, highlighting the need for further research. Novel therapies targeting neuronal hypersensitivity, such as P2X3 antagonists, show promise and warrant further investigation.

Conclusion

Chronic cough is a complex condition with a multifactorial etiology, making its diagnosis and management challenging. A comprehensive, tailored approach is essential for effective management. Future research should focus on elucidating the pathophysiology of chronic cough and developing novel treatment strategies.
