Gestational diabetes mellitus (GDM) poses significant risks to both mother and child. Effective management requires a comprehensive, multidisciplinary approach. This article provides an overview of the latest strategies for optimizing outcomes in GDM.
Early detection and diagnosis are crucial for optimal GDM management. Universal screening, using oral glucose tolerance tests, should be undertaken between 24-28 weeks of gestation. For high-risk patients, screening should begin in the first trimester.
Medical Nutrition Therapy (MNT) is the cornerstone of GDM management. It involves a personalized diet plan focusing on adequate nutrient intake, controlled carbohydrates, and avoidance of excessive weight gain. Regular dietary counseling is beneficial.
Physical activity is recommended unless contraindicated. Moderate-intensity exercise for at least 150 minutes per week, divided into three or more days, has been shown to improve glycemic control.
If MNT and physical activity fail to achieve glycemic targets within one to two weeks, pharmacological therapy should be initiated. Insulin is the first-line drug, but metformin and glyburide are acceptable alternatives.
Postpartum follow-up is essential as women with GDM are at increased risk for type 2 diabetes. Screening should be performed at 6-12 weeks postpartum, and every 1-3 years thereafter. Lifestyle modifications and weight management can reduce this risk.
