Each chronic condition may warrant specific interventions, yet polypharmacy, overlapping pathophysiology, and potential for iatrogenesis complicate management. Prioritization becomes essential when care goals, resource allocation, or therapeutic strategies for one disease may conflict with those for another. This review explores the scientific, clinical, and practical aspects of case-based prioritization, offering insights grounded in recent evidence and guideline recommendations.
Multimorbidity affects an estimated 65% of adults over 65 years and nearly 30% of the general adult population globally. The most common combinations include cardiovascular disease, diabetes, chronic kidney disease, chronic obstructive pulmonary disease (COPD), and depression. The burden is compounded by increased healthcare utilization, higher mortality, diminished quality of life, and substantial economic impact. Epidemiological studies reveal that the presence of two or more chronic conditions exponentially increases the complexity of care, with the risk of adverse outcomes rising with each additional condition.
Chronic diseases often share underlying mechanisms such as inflammation, endothelial dysfunction, neurohormonal activation, and metabolic dysregulation. For example, diabetes and cardiovascular disease are linked by insulin resistance and atherogenic pathways, while chronic kidney disease can exacerbate hypertension through volume overload and altered renin-angiotensin-aldosterone system activity. Major clinical guidelines increasingly recognize the importance of case-based prioritization in multimorbidity. The American Geriatrics Society, NICE, and the American College of Physicians emphasize the need to move beyond disease-centric approaches, advocating for comprehensive assessments, integration of care, and alignment with patient goals. Guidelines recommend regular medication reviews, deprescribing where appropriate, and the use of validated tools for frailty and functional status assessment.
