Abstract

Vitiligo is a chronic depigmenting disorder characterized by the loss of functional melanocytes and subsequent development of white patches on the skin. While a range of medical therapies are available, surgical intervention, particularly skin grafting, offers hope for stable, refractory cases. This review synthesizes current evidence on the role of skin grafting techniques in vitiligo management, analyzes patient selection criteria, elucidates mechanistic underpinnings, discusses clinical outcomes, addresses risks, and summarizes recent advances and consensus guidelines to aid clinicians in decision-making.

Introduction

\n

Vitiligo affects approximately 0.5–2% of the global population, manifesting as progressive, well-defined depigmented macules and patches. Despite significant advancements in medical therapy—including topical corticosteroids, calcineurin inhibitors, and phototherapy—many patients experience persistent lesions unresponsive to conventional approaches. Consequently, surgical modalities, with skin grafting at the forefront, have emerged as viable alternatives for carefully selected patients with stable vitiligo. This review critically examines the scientific rationale, methodology, and clinical implications of skin grafting in the management of vitiligo, with a focus on recent evidence and international guideline recommendations.

Epidemiology / Disease Burden

\n

Vitiligo imposes a considerable psychosocial and quality-of-life burden, with prevalence estimates ranging from 0.5 to 2% globally. Both genders are affected equally, and the disorder often presents before the age of 20. The stigma associated with visible depigmentation can lead to psychological distress, social withdrawal, and reduced self-esteem, underscoring the importance of effective and timely interventions. Notably, up to 25% of cases may demonstrate resistance to medical therapy, highlighting the clinical need for alternative treatment modalities such as surgical grafting.

Pathophysiology

\n

The pathogenesis of vitiligo is multifactorial, involving genetic predisposition, autoimmune mechanisms, oxidative stress, and neurogenic factors. The primary event is the destruction or functional impairment of epidermal melanocytes, resulting in localized or generalized depigmentation. Stable vitiligo, defined as the absence of new lesions or expansion of existing lesions for at least 6–12 months, is a critical prerequisite for surgical intervention, as ongoing disease activity can compromise graft survival and repigmentation outcomes.

Risk Factors

\n

Risk factors for vitiligo include a family history of autoimmune diseases, personal history of thyroid dysfunction, type 1 diabetes, and other pigmentary disorders. Environmental triggers, such as skin trauma (Koebner phenomenon), sunburn, and psychological stress, may precipitate or exacerbate disease activity. Understanding these risk factors is essential for patient selection and perioperative risk stratification prior to surgical intervention.

Clinical Features

\n

Vitiligo is characterized by sharply demarcated, milky-white macules and patches that may coalesce into larger areas of depigmentation. Commonly affected sites include the face, hands, forearms, feet, and genitalia. Mucosal involvement, leukotrichia (white hair), and segmental or non-segmental patterns are observed. The stability of lesions—marked by the absence of new or expanding patches—is paramount for considering surgical procedures such as skin grafting.

Diagnosis

\n

The diagnosis of vitiligo is primarily clinical, supported by a thorough history and physical examination. Wood\"s lamp examination accentuates depigmented areas, aiding in disease delineation. In uncertain cases, histopathology reveals an absence of melanocytes in affected skin. Preoperative assessment for surgical intervention should include evaluation of disease stability, exclusion of active inflammation, and patient expectations.

Treatment & Management

\n

First-line therapy for vitiligo comprises topical corticosteroids, calcineurin inhibitors, and narrowband UVB phototherapy. These modalities are most effective in early or active disease. For refractory, stable cases, surgical options such as tissue grafts (split-thickness, punch, suction blister) and cellular grafts (non-cultured epidermal suspension, cultured melanocyte transplantation) are considered. Skin grafting involves the transfer of melanocyte-containing tissue from normally pigmented donor sites to depigmented recipient areas, promoting repigmentation through melanocyte proliferation and migration. Careful patient selection, meticulous technique, and postoperative care are crucial for optimal outcomes.

Recent Advances / Emerging Therapies

\n

Recent years have witnessed innovations in grafting techniques, including the use of non-cultured epidermal cell suspensions, which offer shorter procedural times and the ability to treat larger areas. Platelet-rich plasma (PRP) and growth factor-enriched matrices are being investigated as adjuncts to enhance graft take and melanocyte survival. Innovations in laser-assisted dermabrasion and minimally invasive harvest methods have improved safety and reduced donor site morbidity. Ongoing trials are evaluating the synergistic effects of combining surgical grafting with phototherapy and immunomodulators.

Guideline Recommendations

\n

International guidelines, including those from the Vitiligo Global Issues Consensus Conference and European Dermatology Forum, recommend surgical intervention only for patients with stable, refractory vitiligo unresponsive to medical therapy. Key prerequisites include disease stability for at least 6–12 months, absence of keloidal tendency, and realistic patient expectations. Skin grafting is contraindicated in active, rapidly progressive disease or in patients with a high risk of hypertrophic scarring. Multidisciplinary assessment and counseling are advised to optimize patient satisfaction and procedural success.

Conclusion

\n

Skin grafting represents a scientifically validated, guideline-endorsed surgical option for select patients with stable, treatment-resistant vitiligo. While surgical intervention can yield excellent repigmentation and significant psychosocial benefits, careful patient selection, rigorous preoperative evaluation, and adherence to technical best practices are essential to maximize success and minimize risk. Continued research into adjunctive therapies and novel grafting techniques holds promise for further improving outcomes in this challenging disorder.
