Vertigo, characterized by a sensation of perceived motion, is a common clinical challenge faced by healthcare professionals. It is often a symptom of various underlying disorders, primarily those affecting the vestibular system. This article aims to provide a comprehensive clinical approach towards diagnosing and managing vertigo.
Vertigo can be peripheral or central, based on the site of lesion. Peripheral vertigo, the most common type, is often due to conditions such as benign paroxysmal positional vertigo (BPPV), Meniere's disease, or vestibular neuritis. Central vertigo, though less common, can be a sign of serious disorders like stroke or multiple sclerosis.
Clinical evaluation begins with a detailed history, focusing on the onset, duration, frequency, and associated symptoms of vertigo. Physical examination should include a thorough neurological and otological assessment. Special tests like the Dix-Hallpike maneuver for BPPV and head impulse test for vestibular neuritis can be invaluable.
Imaging studies and vestibular function tests are often necessary to confirm the diagnosis. Audiometry can detect associated hearing loss in Meniere's disease. MRI brain is crucial to rule out central causes. Electronystagmography (ENG) and videonystagmography (VNG) can help assess vestibular function.
Treatment depends on the underlying cause. BPPV can be managed with canalith repositioning maneuvers. Vestibular suppressants are useful in acute episodes but should be avoided long term due to potential side effects. In Meniere's disease, a low-sodium diet and diuretics can be beneficial. Surgical interventions may be considered for refractory cases.
Vertigo, while often benign, can be a symptom of serious underlying disorders. A systematic approach, combining clinical assessment with appropriate diagnostic modalities, can help healthcare professionals accurately diagnose and manage this condition. Continued education and research in this field are essential to improve patient outcomes.
