Enteric fever, primarily caused by Salmonella Typhi and Paratyphi, remains a significant global health concern. Despite advances in diagnostic methods and treatment, it continues to pose challenges due to increasing antimicrobial resistance. This article provides an updated approach for the diagnosis and treatment of enteric fever.
Early diagnosis is crucial for effective management of enteric fever. The gold standard for diagnosis remains blood culture, which is most effective during the first week of fever. However, its sensitivity can be compromised by prior antibiotic use. Molecular techniques such as PCR are increasingly being used due to their high sensitivity and rapid turnaround time. Serological tests, though less sensitive and specific, may still be useful in resource-limited settings.
Antibiotic therapy remains the cornerstone of enteric fever treatment. The choice of antibiotic should be guided by local susceptibility patterns, with ceftriaxone and azithromycin being the first-line treatments in areas with high rates of fluoroquinolone resistance. In severe cases, supportive care including hydration and antipyretics is essential. Vaccination should also be considered in endemic areas or for travelers to such regions.
The emergence of multidrug-resistant (MDR) and extensively drug-resistant (XDR) strains of Salmonella Typhi has complicated treatment. Regular surveillance and antimicrobial stewardship programs are vital to manage this threat. Rapid molecular diagnostics can also aid in the early detection of resistant strains, allowing for appropriate treatment adjustments.
Despite advancements, enteric fever remains a significant health issue, especially in developing countries. Early diagnosis, appropriate antibiotic therapy, and proactive management of antimicrobial resistance are crucial in the treatment of this disease. Healthcare professionals must stay updated with the latest guidelines to effectively manage enteric fever and mitigate its global impact.
